📄 📘 Copies of all passport pages (3 copies)
📄 🖨 Printed, signed & barcoded application form (3 copies)
📄 🧾 Curriculum Vitae (3 copies) — sample available
📄 🌍 Proof of permanent residence abroad (Green Card or equivalent) — 3 copies

👨‍👩‍👧 Notarized consent statement (parents / spouse / ex-spouse):

  • 📑 1 original + 2 copies
  • 🏢 May be certified at the Consulate if required

📄 🎂 Birth certificate copy (3 copies)
📄 👶 Children’s birth certificates (notarized + apostilled/legalized if abroad) — 3 copies
📸 🖼 6 passport-size photos (3.5 x 4.5 cm)
📄 💍 Marriage and birth certificates (3 copies)
📄 👦 Documents for children aged 14–18 (if applicable)
📄 🪪 Spouse passport copy (3 copies)
📄 📒 Employment record book copy (if available) — 3 copies

💵 💰 Consular fee: 30 USD
📬 Money order payable to: “Uzbekistan Consulate in WA”
🚸 Children under 18 — ❌ exempt from payment


⚠️ 3. Important Notes

🪪 Only valid biometric passport holders may apply
❌ Lost passport holders cannot apply
🌐 Foreign citizens are not eligible for this service

📑 Documents issued in Uzbekistan may be certified at the Consulate

⏳ Processing time: 3–6 months
🚫 Consular fee is non-refundable even if refused
🏛 Applicant must apply in person at the Consular Section
