Documents may be submitted by mail.<\/p>\n<h4>1. Application Form<\/h4>\n<ul>\n<li>3 printed and signed copies of the application form;<\/li>\n<li>Complete the application online at:<br \/>\nhttp:\/\/consulate.mfa.uz\/anketa-new\/index<\/li>\n<li>A JPG photograph (up to 90 KB) must be uploaded.<\/li>\n<\/ul>\n<p>\ud83d\udccc For minors, the child\u2019s information and parents\u2019 details must be included.<\/p>\n<h4>2. Photographs<\/h4>\n<ul>\n<li>2 color photographs;<\/li>\n<li>Size: 50\u00d750 mm (2\u00d72 inches).<\/li>\n<\/ul>\n<h4>3. Passport<\/h4>\n<ul>\n<li>Original expired passport;<\/li>\n<li>One photocopy of the passport;<\/li>\n<li>Main information page;<\/li>\n<li>Registration (propiska) page;<\/li>\n<li>OVIR page.<\/li>\n<\/ul>\n<h4>4. Birth Documents<\/h4>\n<p>For persons born in Uzbekistan who never received a passport:<\/p>\n<ul>\n<li>Copy of birth certificate;<\/li>\n<li>Copies of parents\u2019 passports.<\/li>\n<\/ul>\n<h4>5. Minors (Under 18)<\/h4>\n<ul>\n<li>Birth certificate;<\/li>\n<li>Passport (if available);<\/li>\n<li>Copies of parents\u2019 passports.<\/li>\n<\/ul>\n<h4>6. Children Born in the U.S. or Canada<\/h4>\n<ul>\n<li>Birth certificate (original and copy);<\/li>\n<li>Apostille;<\/li>\n<li>Copy of Green Card or foreign passport.<\/li>\n<\/ul>\n<h4>7. Proof of Legal Status<\/h4>\n<ul>\n<li>Copy of Green Card; or<\/li>\n<li>Copy of foreign passport.<\/li>\n<\/ul>\n<p>This document is mandatory.<\/p>\n<h4>8. Consular Fees<\/h4>\n<ul>\n<li>Age 16 and older: $60 USD<\/li>\n<li>Under age 16: $10 USD<\/li>\n<\/ul>\n<p>Money Order payable to:<br \/>\n<strong>Uzbekistan Consulate in WA<\/strong><\/p>\n<h4>9. Validity of the Certificate<\/h4>\n<ul>\n<li>Issued for 1 month or 3 months;<\/li>\n<li>Cannot be extended;<\/li>\n<li>Applications submitted more than 3 months before travel will not be accepted.<\/li>\n<\/ul>\n<h4>10. Passport Requirements<\/h4>\n<ul>\n<li>Expired passports must be submitted;<\/li>\n<li>Passports are forwarded to the internal affairs authorities in Uzbekistan;<\/li>\n<li>Processing may take approximately 1.5\u20132 months.<\/li>\n<\/ul>\n<h4>11. Lost Passport<\/h4>\n<ul>\n<li>Police report (if available);<\/li>\n<li>Official document confirming passport loss issued by U.S. or Canadian authorities.<\/li>\n<\/ul>\n<h4>12. Important Rules<\/h4>\n<ul>\n<li>A separate certificate is issued for each individual;<\/li>\n<li>Information about the accompanying adult must be included for children under 16.<\/li>\n<\/ul>\n<h4>13.
